The Population Health Alliance is the premier association for the professionals of organizations that provide and support population health. PHA Membership is open to all stakeholders in the population health improvement community.

PHA Membership is now open to all For-Profit Companies and national Non-Profit Organizations which have an interest in population health management. We have recently streamlined our membership application process reducing the number of membership categories offered. PHA membership is still built upon a tiered dues structure based upon the company or organization’s annual revenues, with two exceptions
- Contributing Companies* (such as Employer is NOT selling population health solutions, Accountable Care Organizations (ACOs); Integrated Care Networks (ICNs) or Integrated Delivery Networks (IDNs); hospitals and/or physician groups, such as private equity, venture capital, or other nonprofit organization) – PHA annual dues for these entites are $2,500.
- Health Plan Departments or Divisions** (such as single state, a Medicare division or innovation department) – PHA annual dues are $5,000.

Membership Application: Less than $1 million – $1,000
Membership Application: $1 million and less than $5 million (or Contributing Company*) – $2,500
Membership Application: $5 million and less than $25 million (or Health Plan Dept/Division** – $5,000
Membership Application: Greater than $100 million – $20,000
Membership Application: $25 million and less than $100 million – $10,000
The Population Health Alliance will provide an invoice for all methods of payment, including credit card payments.
We do not accept purchase orders. Payment must be submitted within 15 days of application submission.
*Membership dues are neither transferable nor refundable. While 1% of dues is non‐deductible for federal income tax purposes,
all or part of the remainder may be deductible as a business expense; please consult your tax adviser
