Our purpose in writing this article is to highlight how Population Health Management (PHM) strategies apply to and integrate with the lives of people over the age of 50, applying our framework for supporting healthy lives. We follow these individuals along the aging continuum—moving with the individual from being active and well, to becoming chronically ill or possibly frail disabled, or as they age with optimal health in mind and body.
