The Population Health Alliance is the preeminent association for the professionals of organizations that provide and support population health. PHA Membership is open to all stakeholders in the population health improvement community.

- Purchasers and Investment Firms ‐ Companies that typically purchase population health management programs such as Employers**, Union groups, TPAs or those that invest in Population Health Management companies such as Private Equity Firms, Angel Investors, Investment Banks
- Provider Groups — Includes organizations such as integrated delivery networks, patient centered medical homes, Accountable Care
Organizations, health systems (hospitals), IPAs and medical group practices
- Other Non‐Profits — Regional nonprofit health plans, non‐profit association and quality organizations, and community organizations

Tiered dues are assessed at the greater of annual revenue of the parent company, the population health management company, or health plan.

The Population Health Alliance will provide an invoice for all methods of payment, including credit card payments. We do not accept purchase orders. Payment must be submitted within 15 days of application submission.
*Membership dues are neither transferable nor refundable. While1% of dues is non‐deductible for federal income tax purposes, all or part of the remainder may be deductible as a business expense; please consult your tax adviser.
