The Solutions Zone Walking Learning Labs

This year, CCA Forum will have another learning opportunity for attendees and yet one more avenue for exposure for our exhibitors: the Walking Learning Labs. These labs intent to answer the question: how does your company implement a population health management solution and what would be the value on investment? Attendees will pre-register for the Labs and will be guided through them by CCA researchers, who will introduce the topic, set up a series of questions and walk the exhibit hall, The Solutions Zone, with the attendees to visit four or six selected booths to provide hands on demonstrations. Pre-register now.

Analytics and Predictive Modeling for Risk Stratification: How does it work

Overview

Using information collected in health assessments, predictive modeling stratifies individuals into meaningful categories for personalized intervention targeting.

Stratification should include categories that represent the continuum of care in the population. While some organizations use complicated and proprietary mathematical algorithms to predict risk, others use a simple count of risks to classify individuals.

Whatever the process, it is important for population health management to include some type of stratification to help align individuals with appropriate intervention approaches and maximize the impact of the program.

Company Profile

ActiveHealth Management

ActiveHealth’s patient portal, MyActiveHealthSM, is an online engagement platform with an integrated health assessment built on a foundation of analytics and predictive modeling.

The underlying analytics ingest HA and other available data to create a unique health profile and perform risk analysis, MyActiveHealth then serves the user the most appropriate information and tools for improving their health through this personalized portal.

A demonstration of how this platform behaves depending on three unique individual profiles across the health spectrum will show how risk stratification can be used to effectively target and engage people online.

Aerocrine

Aerocrine would demo NIOX MINO®, a medical device that, in a 10-second POC breath test, measures the Nitric Oxide level in the patient’s airways. This FDA-cleared, easy-to-use, non-invasive tool provides physicians with an inexpensive, objective way of evaluating allergic airway inflammation (asthma).

It helps identify patients who do/don’t have asthma, guides treatment by identifying patients that will/won’t respond to inhaled steroid therapy, and allows physicians to monitor the patient’s progress and adherence to prescribed therapy. Used in chronic disease management of asthma, FeNO testing helps decrease ER visits and hospitalizations, and while enabling more cost-effective utilization of medication.

Population Health Management in Provider Models: How does it work?

Overview

The importance of embedding population health management strategies into primary care centered models is increasing as primary care and multispecialty practices transform to meet the new demands of health care reform.

Practices that can successfully embed these strategies as well as measure the impact will benefit as they demonstrate improved health of their patients while reducing costs. Primary care centered models can transform their practices to include the components of identification, health assessment, risk stratification, patient-centered interventions, and impact evaluation.

Company Profile

Innovative HITECH Healthcare Solutions

Our CARES platform gives each provider or small medical group their own personalized brand and customizable app at an affordable cost; making it a viable solution to the ACA 2014. A study from England reports a 24% higher cost in socially isolated individuals than for socially connected individuals with an equivalent level of clinical risk.

The flexibility of the CARES platform allows a customizable interface to provide disease-focused or person-focused communication channels. By designing focused pathways from behavioral insights, with claims data for a specific healthcare area, subgroup of patients can be identified before high-cost episodes occur.
